29 CFR  1910.95(g)(1):The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  At the commercial facility located at 6439 State Hwy 43 in Joplin, Missouri 64804, on or about 04/29/2025:  The employer did not establish a baseline audiometric testing for work areas or tasks where employee exposure equaled or exceeded the action level of 85 decibels measured on the A-scale.  Employees were exposed to continuous noise levels higher than 85 dBA in the following areas:  a) Welding Bay 2 - An employee was exposed to continuous noise level at 78.8% dose of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 87.9 dBA. The sampling was performed for 475 minutes during one shift.

29 CFR  1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  At the commercial facility located at 6439 State Hwy 43 in Joplin, Missouri 64804, on or about 04/29/2025, and at times prior: Employees were exposed to respiratory hazards that included but not limited to leakage of contaminants, unreliable fit, asthma, skin irritation, dermatitis, and other medical conditions, while wearing tight-fitting respirators with facial hair.  Wearing tight-fitting elastomeric respirators with facial hair contributes to a poor seal and effectiveness of the respirator.
