29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about April 30, 2025, the employer did not ensure that the Auto Mold rotating section (Work Center 14) was guarded to protect employees from pinch point and struck by hazards in that the equipment could be restarted from the control stand while employees were still inside the operational area. b) On or about April 30, 2025, the employer did not ensure that guarding was provided at the in running nip point where the rollers meet the front of the Happi 1 and Happi 2 machines (Work Center 8), exposing employees to caught-in hazards.

29 CFR 1910.303(g)(1): Electrical. General. Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a) On or about April 30, 2025, the employer did not maintain the minimum required working space around electrical panels as required by 29 CFR 1910.303(g)(1) and Table S-1. Specifically, 480-volt, 208-volt, and 220-volt panels were obstructed by equipment, cabinets, and tool organizers, preventing safe access and exposing employees to electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a) On or about April 30, 2025, in the Maintenance Area, electrical cords entering 480-volt electrical panels located on the right-side wall were not provided with strain relief. As a result, pull on the cords could be directly transmitted to the terminal connections, exposing employees to electrical hazards.
