1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. On or about April 25, 2025, the employer did not ensure that powered industrial lift trucks were operated in a manner to maintain approximately three truck lengths from the truck ahead as required. As a result, an employee operating a powered industrial vehicle had his view obstructed and did not see that an employee was in front of him. The pallet on the on the forks of the powered industrial vehicle struck the injured employee's lower right leg causing an injury.

29 CFR 1910.178(n)(6): Industrial truck drivers shall be required to look in the direction of, and keep a clear view of the path of travel: On or about April 25, 2025, the employer failed to ensure an employee operating a powered industrial vehicle kept a clear view of the path of travel. As a result, an employee operating a powered industrial vehicle had his view obstructed and did not see that an employee was in front of him. The pallet on the on the forks of the powered industrial vehicle struck the injured employee's lower right leg causing an injury.
