29 CFR  1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  a) On or about and at times prior to May 6, 2024, at the establishment located at 808 Packerland Drive, Green Bay, Wisconsin; in the Kettle Room and Old Kettle Room, the employer did not select and require the use of personal protective equipment that would protect each employee from the hazards identified in the hazard assessment, such as but not limited to appropriate torso/arm protection, exposing employees to thermal hazards from extremely hot liquids.

29 CFR  1904.4(a): Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that:  The Kettle Cuisine LLC - 808 Packerland Drive, Green Bay, Wisconsin. On or about and at times prior to July 21, 2025, the employer failed to record work related illnesses on the OSHA 300 Log for calendar year 2025 that met recordable criteria in the following instances:  a) On or about April 4, 2025, an employee was moving buckets off of the 16 pound line when they felt pain in their right knee, resulting in an other recordable case.  b) On or about April 16, 2025, an employee was scraping and stacking trays in the bone room when they felt a pain in their arm, resulting in job transfer or restriction. As a part of the employee's normal job duties, they were required to lift weight greater than their restrictions, such as but not limited to boxes of bones.   c) On or about May 27, 2025, an employee sustained a repetitive motion injury, resulting in job transfer or restriction. As a part of the employee's normal job duties, they were required to lift weight greater than their restrictions, such as but not limited to boxes of bones.   d) On or about July 11, 2025, an employee sustained a right foot injury after slipping off a pallet, resulting in job transfer or restriction. As a part of the employee's normal job duties, they were required to lift weight greater than their restrictions, such as but not limited to boxes of bones.

29 CFR  1904.7(b)(4): How do I record a work-related injury or illness that results in restricted work or job transfer? When an injury or illness involves restricted work or job transfer but does not involve death or days away from work, you must record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column.  The Kettle Cuisine LLC - 808 Packerland Drive, Green Bay, Wisconsin. On or about and at times prior to July 21, 2025, the employer did not enter the number of calendar days restricted work or job transfer on the OSHA Form 300 or equivalent for the calendar year 2025.  a ) On or about February 27, 2025, an employee sustained a strain/contusion of lower right leg after tripping on uneven floor (Drain hole cover out of place) (Case Number 2), which resulted in a job transfer or restriction case for 19 calendar days. The employer's OSHA 300 Form or equivalent for the calendar year 2025 did not provide the number of calendar days restricted work or job transfer on the OSHA Form 300 or equivalent for the calendar year 2025.
