29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employer does not protect each employee from fall hazards while performing roofing activities on a commercial building with unprotected edges at a height of approximately 18.3 feet above a lower level by the use of guard rail systems, safety net systems, or personal fall arrest systems. This most recently occurred on May 7, 2025, at 5800 Everhard Rd., Canton, Ohio 44718. Employees are exposed to the hazard of falling approximately 18.3 feet to the ground, while involved in roofing activities along the unprotected edges of a residential home without using fall protection or a fall protection system.

29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. On or about May 7, 2025, at a work location located at 5800 Everhard Rd. NW, Canton Ohio 44718 the employer, Twin Creek Roofing Services, LLC did not initiate and maintain safety and health programs related to the roofing work that included work rules, hazard detection and site safety enforcement. Employees were performing work on the roof of a commercial building at heights over 18.3 feet without any form of fall protection, which exposed the employees to fall hazards. Twin Creek Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1744124, citation number 1, item number 1 and was affirmed as a final order on 8/13/24, with respect to a workplace located at 8500 Traphagen ST NW, Massillon Ohio 44646.
