29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about May 6, 2025, the employer failed to ensure that the Slitter Machine was guarded at the rear next to the cutting blades/mechanism. Employees are exposed to struck by and/or caught in between hazards when they are pulling materials out through the back of the slitter and can get right up to the slitter blades. b) On or about May 6, 2025, the employer failed to ensure that the #50 SASFI Peg Machine was guarded where the metal grab hooks rotate around the conveyor belt (inside and outside of the belt) to pull materials into the machine. There is also a ram that rapidly moves up and down at the end where the large pins are dropped into a collection bin. The ram itself is not guarded nor is the side component of the ram, which protrudes out towards the employees working station when in motion. Employees are exposed to struck by hazards when working right up against the conveyor or walking underneath it near the moving metal pegs/grab hooks when operating the machine. c) On or about May 6, 2025, the employer failed to ensure that the Roll Deflector RD-1was guarded at the top of the bin where materials are filtering within the moving base and the dropped down a shoot to a collection bin. Employees are exposed to struck by hazards of the moving base when ensuring the bolts are filtering into the base and at the motor and its components that are below and within the drum when the panel is off and they are cleaning out metal shavings and debris.
