29 CFR 1910.137(c)(2)(xii): The employer did not certify that equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. a) On or about May 8, 2025, Electrical room in Deli portion of the warehouse: an employee was exposed to ARC flash hazards, in that, a pair of NOVAX Class 0, 1000 volt rated gloves had not been inspected since December 7, 2023. b) On or about May 8, 2025 , Electrical room in Deli portion of the warehouse: an employee was exposed to ARC flash hazards, in that , a MAGID PowerMaster 12501PS leather lineman glove was not inspected since August 17, 2023. c) On or about May 8, 2025 , Electrical room in Deli portion of the warehouse: an employee was exposed to ARC flash hazards, in that , Chicago Protection Apparel item: SWJ-12-XL protective jacket, was not inspected since March 2, 2021. d) On or about May 8, 2025 , Electrical room in Deli portion of the warehouse: an employee was exposed to ARC flash hazards, in that, Chicago Protective Apparel Model# AMP12RWHTS-EC ARC Flash Protective Shield was not inspected since March 2, 2021.
