29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about May 12, 2025, at the jobsite located at W289N3414 Lost Creek Court, Pewaukee WI, the employer did not ensure that employees utilized the fall protection in place while performing roofing work on a two story residential house with a pitch of 9:12 and a fall height of 11 7/10 feet, thereby exposing the employees to a fall hazard

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about May 12, 2025, at the jobsite located at W289N3414 Lost Creek Court, Pewaukee WI, the employer did not ensure that ladders used to access and egress the two-story residential home with a pitch of 9:12 were extended at least three feet above the landing or secured to prevent displacement when used. Exposing employees to a fall hazard of up to 11 7/10 feet to the ground below.
