29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about May 12, 2025, at the jobsite located at W289N3414 Lost Creek Court, Pewaukee WI, the employer did not ensure that ladders used to access and egress the two-story residential home with a pitch of 9:12 were extended at least three feet above the landing or secured to prevent displacement when used. Exposing employees to a fall hazard of up to 11 7/10 feet to the ground below.
