Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.)  At the worksite, located at 654 W Brandon Blvd Brandon. FL 33511; On or about 5/13/25, the employer exposed employees to fall hazards of approximately 10ft and 14ft, in that, a fall protection system was not provided for employees repairing a roof on a commercial car wash building.

At the worksite, located at 654 W Brandon Blvd Brandon. FL 33511; On or about 5/13/25, the employer exposed employees to fall hazards of approximately 10ft and 14ft, in that, extension ladders were not extended for employees 3ft above the work surface  for employees repairing a roof on a commercial car wash building.
