At the worksite, located at 3414 Bell Shoals Rd Valrico, FL 33596, On or about 1/28/26, the employer exposed employees to fall hazards of approximately 11ft, 16ft and 21ft in that a tubular scaffold that wasn't fully planked was used in order to conduct framing operations on newly built commercial building.

At the worksite, located at 3414 Bell Shoals Rd Valrico, FL 33596, On or about 1/28/26, the employer exposed employees to fall hazards of approximately 11ft, 16ft and 21ft in that a tubular scaffold that did not contain mudsills while conducting framing operations on newly built commercial building.

At the worksite, located at 3414 Bell Shoals Rd Valrico, FL 33596, On or about 1/28/26, the employer exposed employees to fall hazards of approximately 11ft, 16ft and 21ft in that employees were authorized to use cross braces as a means of accessing a tubular scaffold while conducting framing operations on newly built commercial building.
