29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) On or about May 13, 2025, the employer did not ensure that an energy control program was established for the Songu Laser - FLY 4020 Procutter. Specifically, the employer had not developed energy control procedures, provided training for authorized employees, or conducted periodic inspections for employees performing servicing and maintenance tasks, such as making adjustments to the alignment of the red point laser.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about May 13, 2025, the employer did not ensure that the point of operation where employees were working, for machine Songu Laser - FLY 4020 Procutter was guarded in that the door's interlocks were not programmed to stop all movement within the equipment when opened; exposing employees to a caught in hazard from the movement of the laser head. b) On or around May 13, 2025, the employer failed to provide adequate guards around the end table of the Songu Laser - FLY 4020 Procutter. The end table, where parts are positioned for operation, lacked protective surrounding guards, thereby exposing employees to potential caught-in hazards from the movement of the shuttle table as it interacts with the equipment.
