The light curtain used to protect the point of operation was not turned on.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Vision Quality Components on or about May 13, 2025 in the shop: Employees working with Gasbarre 200 Ton Press, Press # 101 are exposed to moving parts at the bottom of the press during automated operation of the machine. The guard at the bottom of the press was in the open position and allowed access to mechanical moving parts of the press. B) Vision Quality Components on or about May 13, 2025 in the shop: Employees working with Gasbarre 45 Ton Press, Press # 105 are exposed to point of operation hazards during press operations on the rear of the machine.
