Far end of the warehouse: The base plates for the storage rack along the wall were not anchored to the floor to prevent movement. Employees were exposed to struck-by and crushed-by hazards. b. Far end of the warehouse: The storage rack along the wall that stored boxes of trash can lines and t-shirts had a bent supporting leg. Employees were exposed to struck-by and crushed-by hazards.
