Warehouse loading dock: On or about June 25, 2025, the employer did not not ensure that the worker loading granite slabs on to a flatbed truck was protected from falling on to the lower level. On the date above, a worker fell through a gap between the dock floor and the truck bed.

Warehouse located at 11631 Fairgrove Industrial Blvd., Maryland Heights, Missouri 63043. On or about June 25, 2025, the employer did not report a work related hospitalization of a worker due to injuries , to the Occupational Safety and Health Administration (OSHA . On the date above, a truck driver fell between the truck bed and the loading dock while loading granite slabs. The worker was hospitalized for several days. The Employer did not report the hospitalization until July 2, 2025.

29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: Location: Warehouse at 11631 Fairgrove Industrial Blvd., Maryland Height, Missouri 63043: On or about May 21, 2025 the following observations were made as related to fork truck use. a) A Hyster fork truck, Model No. S155X12, Serial No. BO24006600X, with damaged seat was not removed from operation until repaired. b) A caterpillar fork truck, Model C6000, Serial No. AT025578, with damaged seat was not removed from operation until repaired.
