29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; and/or on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Low-slope roof (RF-61 & RF-30) on west southwest side of building; On or about May 22, 2025, fall protection was not used while employees walked on, and stood directly at the edge of, a low-slope roof to dispose of shingles and underlayment materials that were removed during commercial reroofing operations.  The flat roof was approximately 54-feet, 10-inches long by 35-feet, 2-inches wide and ranged in height from 17-feet, 10-inches to 19-feet, 11-inches above the ground.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for an employee, and did not comply with all other pertinent requirements of this subpart before that employee began the work that necessitates the fall protection:  (a) Low-slope roof (RF-61 & RF-30) on west southwest side of building; On or about May 22, 2025, the employer did not ensure fall protection systems were provided and/or installed before employees walked on and stood directly at the edge of a low-slope roof to dispose of shingles and underlayment materials that were removed during commercial reroofing operations. 29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system:  (a) Roof of church building; On or about May 22, 2025 and dates prior, roofing employees were permitted to use waist level attachment points at the front/side of body belts (climber style, hip-harnesses) as part of their personal fall arrest systems during reroofing operations of material tear off, roof preparation, and asphalt shingle installation.  The roof slopes varied from 2/12 to 17/12 and the eave height above the ground ranged from 14-feet, 8-inches to 19-feet, 11-inches.  Note: As of January 1, 1998, subpart M of this part (Sec. 1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a positioning device system for vertical surfaces, such as working on a wall, is regulated under Sec. 1926.502(e).
