29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system: (a) West and east side of roof; On or about June 12, 2025, as part of the personal fall arrest system established at the site, employees were wearing climbing style body belts, which went about the waist and attached to a lifeline in the front of the belt. Note: Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: (a) West and east side of roof; On or about June 12, 2025, as part of the personal fall arrest system established at the site, employees were provided synthetic lifeline ropes that were wrapped and knotted around the cap of the roof truss, which was the established anchor point, without a means of protecting the rope from being cut or abraded.
