29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about April 29, 2025, the employer failed to report a finger amputation that occurred on April 28, 2025, within the required 24 hours. An employee sustained a finger amputation in the pully mechanism of the crane and the employer did not report the amputation to OSHA until May 15, 2025.
