29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At the jobsite located at the corner of 3rd St. and Blue Parkway, Lees Summit, MO 64063: an employee was exposed to engulfment and tripping hazards due to inadequate means of egress from an excavation. The portion of the excavation the employee was working in was approximately 6.1 to 6.3 feet in depth. A ladder was not utilized and the employee was required to climb over underground utilities to exit the excavation.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite located at the corner of 3rd St. and Blue Parkway, Lees Summit, MO 64063: An employee was exposed to crushed by and trip hazards while working in a 6 foot trench with no cave-in protection. The employee was measuring the trench for his trench box.
