29 CFR 1926.451(c)(1)(iii):Ties, guys, braces, or outriggers were not installed to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, was applied or was transmitted to the scaffold. On or about May 27, 2025 employees were exposed to caught-by hazard while disassembling a Premier Scaffold Solutions mast climbing scaffold without ties, guys, braces installed to prevent the scaffold from tipping over.

29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee. On or about May 27, 2025 Employees were exposed to fall hazards while dismantling a mast climbing scaffold approximately 16 feet above lower ground level wearing personal fall arrest systems that were not anchored to a point independent of anchorages used to support the mast climbing scaffold.
