29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment. On or about May 27, 2025, on a tree in the backyard of a residential building, an employee was climbing and trimming trees at a height of more than four(4) feet and the hazard assessment did not identify and select a complete fall protection system to maintain continuous fall protection.

On or about May 27, 2025, on a tree in the backyard of a residential building, an employee was not in use of an anchorage under the supervision of a qualified person, as part of the complete fall protection system, while trimming trees at a height of more than four (4) feet.
