29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. On or about May 30, 2025, at the worksite: employees were exposed to an approximate 30 foot fall hazard in that, a job-made wood box was attached to a forklift and used to transport employees from the ground to the work area on the roof.

On or about May 30, 2025, at the worksite: employees were exposed to an approximate 30 foot fall hazard while delivering roofing materials on a 5:12 pitched roof, in that, fall protection was not used.
