29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification. (a): Jobsite - North-East Side - An employee was using a field modified aerial lift without written certification. The employee was using a piece of lumber wedged against the aerial lift in order to detach metal decking above. The aerial lift was modified for a use other than intended by the manufacturer. While standing in the basket, the employee operated the controls to lift the basket to push the lumber into the metal decking; the lumber failed causing the employee to fall from the basket to the ground below, on or about May 6, 2025. There is no abatement certification or documentation required for this item.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a): Jobsite - North-East Side - An employee working from an aerial lift did not wear a body belt or harness and did not have a lanyard attached to the boom or basket. One employee was exposed to fall hazards of approximately 8 feet while working from the aerial lift. The employee fell from the basket of the aerial lift, on or about May 6, 2025. There is no abatement certification or documentation required for this item.
