29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) New commercial construction site located at 5382 Winder Highway Braselton, GA 30517, Scaffold: On or about 06/02/2025 the employer exposed employees to fall hazards of 14-feet, 5-inches to the lower level in that the platform decking was not complete at locations including but not limited to the access/egress point.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired.  a) New commercial construction site located at 5382 Winder Highway Braselton, GA 30517, Scaffold: On or about June 2, 2025, employees were exposed to fall hazards of up to approximately 21 feet to the lower level. The employer did not ensure that damaged scaffold components were repaired, replaced, braced, or removed from service, in that platform planks on the scaffold were significantly dry rotted and exhibited visible longitudinal cracks across most of the deck surface.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   a) New commercial construction site located at 5382 Winder Highway Braselton, GA 30517, Scaffold: On or about June 2, 2025, the employer exposed employees to fall hazards of approximately 14 feet 5 inches to the lower level. Employees were observed climbing the scaffold frame to access and exit the upper levels because the installed ladder did not extend to the second level, and no other approved means of access, such as a properly installed ladder or stair tower, was provided.

29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress.   a) New commercial construction site located at 5382 Winder Highway Braselton, GA 30517, Scaffold: On or about June 2, 2025, the employer exposed employees to fall hazards of approximately 14 feet 5 inches to the lower level. Employees were observed using the scaffold's cross braces to climb and descend between levels, in lieu of a proper means of access or egress, such as a ladder or stair tower, in violation of safe scaffold access requirements.
