29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) New residential construction located at 1603 Azalea Drive, Lawrenceville, GA 30013, Scaffold: On or about 7/2/2025, employees were exposed to struck-by hazards, in that head protection was not provided where overhead masonry activities were taking place.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:   a) New residential construction located at 1603 Azalea Drive, Lawrenceville, GA 30013, Scaffold: On or about 7/2/2025, employees were exposed to struck-by hazards in that toe boards were not provided where overhead masonry activities to include the presence of materials and equipment such as but not limited to trowels and bricks were staged and in use overhead.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) New residential construction located at 1603 Azalea Drive, Lawrenceville, GA 30013, Scaffold: On or about 7/2/2025, the employer exposed employees to fall hazards of 13 feet, 6 inches to the lower level, in that a safe means of access was not provided to each working level of scaffold as employees climbed frames and cross braces.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired.  a) New residential construction site located at 1603 Azalea Drive, Lawrenceville, GA 30043, Scaffold: On or about 7/2/2025, the employer exposed employees to collapse and fall hazards of up to 20 feet to the lower level by failing to remove damaged scaffold components from service, in that scaffold platform planks were significantly dry-rotted and exhibited visible longitudinal cracks across most of their surface, reducing their structural integrity. The employer did not ensure these components were repaired, replaced, braced, or removed from service.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.   a) New residential construction site located at 1603 Azalea Drive, Lawrenceville, GA, GA 30043, Scaffold: On or about July 2, 2025, the employer exposed employees working on the scaffold to fall hazards of approximately 13-feet, 6-inches to the lower level in that guardrails were not were installed on the working levels during masonry activities.
