[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995] 29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: a. Bridge over Yellow Breeches Creek - On or about June 4, 2025, employees engaged in bridge replacement work were working from the bridge, they installed the Frontline Fall Protection portable guard rail system GUS10-YLW , when installing the base plates were positioned sideways to the fall instead of perpendicular to the fall hazard which provides the 200 pounds of resistance, employees were exposed to a fall of greater than 6 feet into the creek below which in some areas were estimated at waist deep and other areas were shallow with rocks protruding from the water.
