(a)HH Construction at East Hackberry Avenue and South Lawler Street, Mitchell, SD 57301: On or about and at times prior to June 4, 2025, the employer did not ensure that employees performing work from or operating the SkyJack 6826 RT scissor lift were trained by a person qualified in the subject matter to recognize the hazards associated with operating the scissor lift being used and to understand the procedures to control or minimize hazards. This condition exposed employees to fall and ejection hazards.
