29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about June 10, 2025, in the CNC Laser area of 554 Colfax St Rochester, NY 14606; Employees were exposed to caught in hazards when Lockout/Tagout procedures were not developed, documented, or utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance on machines such as, but not limited to; cleaning the Cincinnati CL-707 CNC laser.  b) On or about June 10, 2025, in the Press brake area of 554 Colfax St Rochester, NY 14606; Employees were exposed to caught in hazards when Lockout/Tagout procedures were not developed, documented, or utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance on machines such as, but not limited to; changing dies in Amada press brakes.   c) On or about June 10, 2025, in the Tool department of area of 554 Colfax St Rochester, NY 14606; Employees were exposed to caught in hazards when Lockout/Tagout procedures were not developed, documented, or utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance on machines such as, but not limited to; changing wheels on surface grinders.  d) On or about June 11, 2025, in the CNC Router area of 1255 Emerson St. Rochester, NY 14606; Employees were exposed to caught in hazards when Lockout/Tagout procedures were not developed, documented, or utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance on machines such as, but not limited to; setting up Anderson CNC Routers #1, #2, #3, and #4.   ABATEMENT DOCUMENTATION REQUIRED.
