29 CFR  1926.502(d)(15)(i): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   a. On or about June 11, 2025, at the above addressed jobsite, anchorages being used for attachment of personal fall arrest equipment were not installed so that they would be capable of supporting at least 5,000 pounds per employee attached, in that, four employees were attached to the same anchorage point.  Having more than one employee attached to anchorage point, invalidates the manufactures safety factor of at least two  thereby exposing employees to fall hazards.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a.  On or about June 9, 2025, at the above addressed jobsite, employees wearing personal fall arrest systems while performing residential roofing operations were not attached to the lanyard and rope by the D-ring in the center of the employees' backs near should level, thereby exposing the employees to fall hazards.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about June 11, 2025, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
