Employees were exposed to fall hazards of approximately 9 feet and 15 feet.

29 CFR 1926.416(a)(1): The employer did not prevent employee(s) from working in proximity to an electric power circuit that the employee could contact during the course of work and the employee(s) were not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about 06/13/2025 at the site located at 6879 Buckley Road, Syracuse, NY, 13210: The employer did not protect employee(s) from electrical shock when roofers were using multiple tools, including but not limited to a metal scrapers in close proximity to incoming 240-Volt electrical lines.  Van Derhoof Roofing Co, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.416(a)(1), which was contained in OSHA inspection number 1693215, citation number 1, item number 1 and was affirmed as a final order on 07/01/2024, with respect to a workplace located at 152 Wilson Drive, East Syracuse, NY 13057.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) On or about 06/13/2025 at the site located at 6879 Buckley Road, Syracuse, NY, 13210: Employees were exposed to fall hazards when working on a 4:12 and 2:12 roof without the use of fall protection. Employees were exposed to fall hazards of approximately 9 feet and 15 feet.  Van Derhoof Roofing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1693215, citation number 2, item number 1 and was affirmed as a final order on 07/01/2024, with respect to a workplace located at 152 Wilson Drive, East Syracuse, NY 13057.  Van Derhoof Roofing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1680194, citation number 2, item number 1 and was affirmed as a final order on 07/05/2024, with respect to a workplace located at 410 Exeter Street, East Syracuse, NY 132057.
