On or about June 11, 2025, at the above addressed jobsite, an employee conducting masonry operations to include but not limited to; the removal/cutting up of masonry brick that contained Crystalline Quartz Silica, by use of a of a Stihl Partner Saw. On or about June 11, 2025, at the above addressed jobsite, an employee was exposed to falls while performing masonry operations from a tubular welded frame scaffold, seven (7) feet four (4) inches above the ground, that did not have the working level fully planked. Beko Masonry Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.451(b)(1) which was contained in OSHA inspection number 1701531, citation number 1 item number 2 and was affirmed as a final order on January 2, 2024, with respect to a workplace located at 1260 Cavell Ave., Highland Park, IL.
