29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: a) On or about June 23, 2025, the employer provided system used for cave-in protection in a trench that was at a depth of approximately 11.4 feet was not constructed in accordance with the manufacturer's tabulated data as only one hydraulic cylinder was used. Rick's Sewer and Drainage, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(c)(2)(i), which was contained in OSHA inspection number 1499935, citation number 1, item number 1b and was affirmed as a final order on January 1, 2021, with respect to a workplace located at 19 S William St., Mount Prospect, IL 60056. No abatement certification or documentation is required for this item.
