29 CFR 1926.452(w)(6)(v): No employee is on any part of the scaffold which extends outward beyond the wheels, casters, or other supports. On or about June 24, 2025, in the interior portion of the building; employees utilized an electric scissor lift to ascend and descend from the roof were exposed to fall hazards in excess of 12ft while standing on the top rail extending beyond the wheels of the scissor lift.

29 CFR 1926.501(b)(2)(i): Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. On or about June 24, 2025, on the north side of the building; employees engaged in decking operations on or near the leading edge of the roof were exposed to the fall hazards in excess of 6ft.
