On or about August 22, 2025, Midwest Building Concepts, LLC did not ensure employees were protected from fall hazards by the use of guardrail systems, safety net systems, or personal fall arrest systems when working and walking near unprotected edges that were under construction at a jobsite located at 1571 Bryan Road in O'Fallon, Missouri.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, amputation, or the loss of an eye: On or about August 22, 2025, at approximately 9:00 a.m. an employee was hospitalized after falling approximately 18 feet. The employer did not report the injury to the Occupational Safety and Health Administration (OSHA) within the required twenty-four (24) hours. OSHA was notified on August 26, 2025, at approximately 10:00 a.m.
