29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Worksite, Southwest corner: On or about November 8, 2023, an employee was conducting roofing activities between the warning line and the edge of the roof on a commercial building, approximately 18 feet above the ground, and was not protected from falling. The employee was not wearing a harness, nor did he have a lanyard or anchor point. The employee fell of the roof and sustained a left femur fracture, left tibia avulsion fracture, and left side rib fracture. b) Worksite, Northeast side: On or about November 14, 2023, employees were conducting roofing activities between the warning line and the edge of the roof on a commercial building, approximately 18 feet above the ground, and were not protected from falling. The employees were wearing harnesses but did not have a lanyard or anchor point. Classified as Repeat per stipulated settlement agreement, Docket 24-0870.
