29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 26, 2025, at the location of a worksite located at  2079 Revely Ave., Lakewood, OH, 44107: employees were exposed to a fall hazard of approximately 20 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation;  On or about June 26, 2025, during the installation of roofing material at the job site located at 2079 Revely Ave., Lakewood, OH, 44107; The employer did not ensure that employees were provided with or utilized safety eye glasses while using pneumatic nail guns for installing of roof shingles.   MGE Roofing, LLC  was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1589708, citation number 1, item number 1, and was affirmed as a final order on June 15, 2022, with respect to a workplace located at 2684 Claythorne Rd., Shaker Heights, OH 44122.  MGE Roofing, LLC was previously cited for a violation of this occupational safety and health standard  1926.102(a)(1), which was contained in OSHA inspection number 1475112, citation number 01, item number 001 and was affirmed as a final order on July 1, 2020, with respect to a workplace located at 238 West Smith Road, Medina, OH 44256.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 26, 2025, during the installation of roofing material at the job site located at 2079 Revely Ave., Lakewood, OH, 44107; the employer failed to ensure employees working on the roof of a single family home were protected from a fall hazard of approximately 20 feet by utilizing personal fall arrest systems/devices.   MGE Roofing, LLC was previously cited for a violation of this occupational safety and health standard  1926.501(b)(13), which was contained in OSHA inspection number 1589708, citation number 01, item number 002 and was affirmed as a final order on June 15, 2022, with respect to a workplace located at 2684 Claythorne Rd, Shaker Heights, OH 44122.
