29 CFR 1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about June 12, 2025, the employer did not ensure that employees engaged in maintenance activities on the Champion Small End Rotary Envelope Machine (SN : 144), received training in the recognition of applicable hazardous energy sources and the methods and means necessary for isolation and control. Employees were exposed to laceration, crushing and amputation hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Production area: On or about June 12, 2025, the employer did not ensure that employees implemented energy control steps prior to performing service and maintenance activities on the Champion Small End Rotary Envelope Machine (SN : 144). Hazardous energy was not isolated prior to employees clearing envelope jams, exposing them to crushing, laceration and amputation hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) Production Area: On or about June 12, 2025, the employer did not ensure that the Champion Small End Rotary Envelope Machine (SN : 144) was guarded to protect employees from in-going nip points created from a belt and pully system, which could result in crushing, laceration and amputation injuries. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
