29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about June 30, 2025, at a worksite located at 179 N Adams Ave, Berlin, WI 54923, employees were exposed to fall hazards when the employer did not ensure that each employee working from a Snorkel Model TB-A60RDZ Aerial Work Platform wore a harness, with a lanyard attached to the boom or basket of the lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (� 1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under � 1926.502(e).

29 CFR 1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the material listed in 1926.454(a)(1)-1926.454(a)(5). On or about June 30, 2025, at a worksite located at 179 N Adams Ave, Berlin, WI 54923, employees were exposed to fall hazards while working from a Snorkel Model TB-A60RDZ Aerial Work Platform and had not received training to identify fall hazards, methods to mitigate those hazards, and the care and use of fall protection equipment prior to be exposed to the fall hazards.
