Flight of Stairs to Upstairs Office, 625 Sills Road, Genoa, NY 13071, on or about 6/30/2025; The employer did not install a mid-rail on the stair rail system on the open side of a flight of 15 steps leading to an office and meeting room.  b.) Flight of Stairs to Storage Room, 625 Sills Road, Genoa, NY 13071, on or about 6/30/2025; The employer did not install a mid-rail on the stair rail system on the open side of a flight of 14 steps that led to a storage area.
