29 CFR  1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  On and before June 27, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, employees were exposed to potential fall hazards when the employer did not ensure that any of it's forklifts had been marked with with the required information when using a Ballymore Work Maintenance Platform attachment.

29 CFR  1910.178(m)(3): Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized.  On or about February 5, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, the employer did not ensure that employees were not permitted to ride on the forks of a Doosan Model B20T-7, Electrical Forklift while conducting maintenance on an overhead crane above Dip Line #2.

29 CFR  1910.303(b)(6): Deteriorating agents. Unless identified for use in the operating environment, no conductors or equipment shall be located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that have a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures.  On and before July 1, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, the employer did not ensure that a pendant style outlet box on the south side of Dip Line #1 was not exposed to an environment where damp conditions and deteriorating agent were present, causing damage to the housing of the pendant and outlet faces.

On and before July 1, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, employees were exposed to potential fire and shock hazards when a two sided electrical pendant, on the south side of Dip Line #1, was not equipped with strain relief to prevent pull being directly transmitted to the terminal screws.
