On and before June 27, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, employees were exposed to potential fall hazards when the employer did not ensure that any of it's forklifts had been marked with with the required information when using a Ballymore Work Maintenance Platform attachment.

On or about February 5, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, the employer did not ensure that employees were not permitted to ride on the forks of a Doosan Model B20T-7, Electrical Forklift while conducting maintenance on an overhead crane above Dip Line #2.

On and before July 1, 2025, at an establishment located at 1228 West South Park, Oshkosh, WI 54902, the employer did not ensure that a pendant style outlet box on the south side of Dip Line #1 was not exposed to an environment where damp conditions and deteriorating agent were present, causing damage to the housing of the pendant and outlet faces.
