29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: At the temporary job site located at 4734 N. Saker St., Wichita KS 67219, employees engaged in residential roofing activities were exposed burns and shock hazards. Employees were exposed to these hazards in the following instances: 1) Black extension cord plugged into electrical power outlet, and 2) Orange extension cord plugged into black extension cord, in that both extension cords were missing the grounding pin.
