The employee was exposed to amputation hazard while splitting a 36-inch diameter wood log. The caution/danger stickers were faded and not of the correct color.

The employer did not provide an adequately trained person at the work site nor did they adequately use nearby medical centers for treatment for employees working at sites remotely by themselves.
