29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) On or about July 1, 2025, the employer failed to develop, document, and utilize procedures for the control of potentially hazardous mechanical energy during the setup activity of die setting on the D1 wire pointer in the bare copper department. As a result, employees were exposed to hazards related to unexpected start-up.

29 CFR 1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about July 1, 2025, the employer did not ensure that employees operating the D1 wire pointer (Manufacturer- Vaughn Machinery Company, Serial # 9121) were trained as authorized employees with the recognition of of applicable hazardous such as mechanical energy during a die set up and when it's necessary to readjust the wire while on the capstan, and the methods and means necessary for energy isolation and control. b) On or about July 1, 2025, the employer did not ensure that employees operating the D2 wire pointer (Manufacturer- Syncro) were trained as authorized employees with the recognition of of applicable hazardous such as mechanical energy during a die set up and when it's necessary to readjust the wire while on the capstan, and the methods and means necessary for energy isolation and control.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about July 1, 2025, the employer did not ensure that the D1 wire pointer located in the bare copper drawing department had it's rotating parts(the capstan) guarded as the wire pointer was pulling wire through a die with the rotating capstan exposing employees to entanglement and pinch point hazards. b) On or about July 1, 2025, the employer did not ensure that the D2 wire pointer located in the bare copper drawing department had it's rotating parts(the capstan) guarded as the wire pointer was pulling wire through a die with the rotating capstan exposing employees to entanglement and pinch point hazards.
