29 CFR 1910.178(l)(4)(ii)(E): Refresher training was not provided to powered industrial truck operators after a condition in the workplace changed that affected safe operation of the truck: (a) On or about July 7, 2025, the employer did not ensure refresher training was provided to powered industrial truck operators when new conditions arose that affected safe operation of the trucks. The employer relocated into a new workplace/distribution center located in Walton Hills, OH in January of 2025.
