On or about June JO, 2025; at the super absorbent  polymer (SAP) unit, where the rotary valve RV -3535A was not turned off or shut down by an employee that broke the pipe I foe that connected to the rotary valve with a body part as required by the procedure for Line Clearing Through Tsubaki Doors.
