The street was divided into two eastbound lanes, a middle lane, and two westbound lanes. The furthest south westbound lane was closed with traffic control devices; however, the two eastbound lanes had no traffic control devices installed along the work area to protect workers. The traffic control plan was not followed in that it specified traffic control device installation in this area. This exposed employees to struck-by hazards from motor vehicle traffic.
