OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of racking systems tipping over or collapsing. a) Shipping Warehouse, on or about July 09, 2025, employees use forklifts to load and unload material, on and from multi-tiered racking systems which are not anchored to the floor and do not have load plaques identifying permissible load capacities. Employees are exposed to the hazard of the racks tipping or collapsing if overloaded or struck by a forklift. NOTE: Abatement certification is required for this item.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: a) Hallway Kitting line 3, on July 09, 2025, employees were exposed to struck by or crushing injuries from powered industrial truck operations while walking through high traffic areas. Employees routinely walk the hallway to their workstations while powered industrial trucks are moving material. b) Shipping Warehouse, on July 09, 2025, employees were exposed to struck by or crushing injuries when working or walking in aisle 510 due to the storage of stacked empty pallets narrowing the forklift path. NOTE: Abatement certification and documentation is required for this item.
