29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) North and south side of roof; On or about July 8, 2025, the employer did not enforce the use of safety glasses with employees using pneumatic nail guns on roof shingles to protect them from flying particles from the nailing process.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Throughout roof; On or about July 8, 2025, while setting and nailing new roofing materials on a 5-12 pitch roof, workers were not protected from falling off the roof by a fall protection system while exposed to falls ranging from 8-feet up to 18-feet above the ground.
