29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Northwest side of residential structure, ground-level; On or about April 7, 2026, a 13-foot Gorilla brand aluminum multi-position ladder, configured as an extension ladder and used for accessing and exiting the roof, was not set-up with the side rails extending at least three feet above the upper landing surface and/or was not secured from displacement.
