29 CFR  1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  (a)  Ground around house - On or about July 8, 2025, the employer exposed employees to trip and fall hazards, in that boards, including boards with nails, were not kept cleared where employees were required to work.

29 CFR  1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.   (a)  In front of the house - On or about July 8, 2025, the employer exposed employees to fall and struck-by hazards, in that the employer did not take out-of-service a JLG GS 42A telehandler forklift that no longer had a functional seatbelt.

(a)  In front of the house - On or about July 8, 2025, the employer exposed employees to potential  struck-by and crushed-by hazards, in that an employee operated a forklift without reevaluation at least every 3 years.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a)  Roof: On or about July 8, 2025, the employer exposed employees to a fall hazard, in that fall protection was not used by employees decking a roof with an 18' fall to ground distance.  Jose Briceno was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1519607, citation number 1, item number 1 and was affirmed as a final order on May 17, 2021, with respect to a workplace located at 100 Telford Street, Savannah, GA 31407.  Jose Briceno was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1744126, citation number 1, item number 2 and was affirmed as a final order on November 18, 2024, with respect to a workplace located at 206 Foster Street, Rincon, GA 31326.
