29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) 15200 Kutztown Road Kutztown, PA, Johnson Hall: On or about July 11, 2025, employees were exposed to fall hazards while conducting construction demolition work, as the employer did not ensure that a floor hole cover was securely installed. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.34(b):Exits and access to exits were not marked by a readily visible sign: a) 15200 Kutztown Road Kutztown, PA, Johnson Hall: On or about July 11, 2025, Employees were exposed to the hazard of delayed egress during an emergency because exits on the Third floor (4th Story) were not marked with readily visible signs, where the route to the exit was not immediately apparent. b) 15200 Kutztown Road Kutztown, PA, Johnson Hall: On or about July 11, 2025, Employees were exposed to the hazard of delayed egress during an emergency because exits on the Fourth floor (5th Story) were not marked with readily visible signs, where the route to the exit was not immediately apparent. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
